Hospital Passport

This is blank template of a Hospital Passport for autistic people and people with a learning disability whose local hospital is GWH. This Hospital Passport contains the contact details for the Learning Disabilities and Autism Liaison service for Great Western Hospital, so that patients can seek support from a department within the hospital.

Any one who has a diagnosis, are self-diagnosed or even awaiting diagnosis are welcome to use this document to help you to receive the best possible care whilst attending GWH.
